If you're planning on having children between now and becoming an attending, particularly while you're still in school, you'll probably want to go with the low deductible plan. Yes, getting the free $1,321.56/yr is worth having an HSA and no, you don't need to worry about contributing while you're in such a low tax bracket, but having a couple of babies will make that low $1,800 family deductible very appealing. Plus the HSA has an unlimited lifetime maximum - scary when you're in childbearing years.
