$1330 per month for an Aetna high deductible plan. Employer pays 75% of the premium, so we pay $335 per month. Aetna puts $125 per month into the HSA, so effectively this is $210 per month from us.

Plan includes basic dental and vision, which is nice.

About $1000/mos for family of 6, BCBS HMO plan, deductible 4K. Annoying thing is it would be only about $300/mos if spouse was willing to go back to full time. Part timers really dinged there. But still way cheaper than my private practice plan.

$449/mo through Liberty Healthshare for a family of 6 with a $1500 "deductible". This is our first year trying a healthshare program.  To buy insurance again for our family would have been ~$1200/mo.

$97/month family of 5 on a HDHP with a $3k deductible. HSA managed by CYC let’s me invest in TDA with HSA Bank as intermediary.

Getting jacked up to $120/month for 2018 with a separate dental charge of $20/month that’s bundled with my current premium.

                Employer subsidized, through UHC.
