This is the major problem with them – I see anecdotally overutilization of resources by NPs/PAs to make up for lack of knowledge. The Mayo study supported this belief. Unfortunately, too many studies are out there that show no difference in the quality of care provided. This has been the major strategy failure on behalf of physicians in regards to mid levels – to prove their value. It really shouldn’t be that hard. And if it is then we need to rethink a lot from educational pathways to salaries.

Agree. NPs from what I see love to churn out consults for Every. Little. Thing. If you come in with any complaint, you will get a consult. Makes more money for the hospital system I guess from over testing and unnecessary consults. I’m wondering if number of tests/consults a patient gets factored into the quality of care metrics, and if so, how. Unfortunately, the patient is often given inaccurate info from the start from the NP regarding some of their testing/imaging and I have to spend at least half of the consult undoing that damage and reeducating.
Hospitalists and PCPs just need to figure out how to make themselves available as consultants. Trust me, they will consult you. And there will always be people like me who will insist on seeing a physician.

I think a good start would be to ban the term “PCP”. I want a primary care physician, not a primary care “provider”. It’s amazing the hoops you have to jump through to get a med school graduate instead of an NP in some large group settings like the VA.
Besides, PCP is what that violent guy in the ER is high on at 3 in the morning.

No, I mean an actual consultant. The supervising physician is often left to sign off on the chart for a patient they didn’t have the benefit of actually seeing. I don’t know why any doctor agrees to that set up. If they are allowed to practice independently, the NP won’t have that mental safety net when they sign the chart and will feel the need to consult someone with any patient who is the least bit complex/not responding to their usual algorithm. No reason the internists/FPs can’t join the rest of us in taking care of those (often easy) consults, especially for those things that are solidly within their scope of practice.
